Stable Housing as the Foundation for Mental Health Recovery

Guild helps individuals experiencing chronic and long-term homelessness find stable, affordable housing in the community of their choice. Then, we provide supportive services to help them remain in their residence, engage in their community, and reach their personal goals.

Housing Support

This program provides ongoing supportive services and assistance with accessing resources for individuals and families who have been referred through the Coordinated Entry System. It is designed to provide the stability necessary to engage in the community and successfully pursue broader health and employment goals. This is a scattered site housing program that helps clients to identify housing in Dakota, Anoka, Scott, or Ramsey Counties. Referrals come from the Coordinated Entry Systems in these counties (SMAC and Ramsey County Coordinated Entry) and direct referrals from providers. At Guild, we use a Long Term Homeless (LTH) model in our Housing Support program to provide rental administration and supportive services.

Guild-Hearth Program

This program is a collaboration with Hearth Connection that provides case management services for individuals who meet long-term homeless criteria. The case management services provide supports to prepare individuals for a housing subsidy, with ongoing support to navigate successful tenancy and connections to long-term community supports.

Delancey Apartments

This 13-unit building in St. Paul, a collaboration with Project for Pride in Living and the St. Paul Public Housing Authority, provides permanent supportive housing for chronic and long-term homeless individuals. Residents enjoy a supportive community and convenient access to resources on a bus line. On-site staff assist with tenancy education and support along with connections to resources in the community.
