Guild offers coordinated support to stabilize individuals’ lives, wherever they are. Our teams connect individuals to vital resources, empower them to manage their mental and physical health needs, and help them navigate social systems. Programs like Assertive Community Treatment (ACT) and Targeted Case Management (TCM) provide intensive, mobile services to prevent crises and improve overall well-being.
Community Access
Community Access connects clients to community-based resources for support and to prevent mental health problems from escalating into a crisis. It begins with a phone screening or online form to assess needs, with a goal of referrals to ongoing services, whether through Guild or elsewhere.
ACT - Dakota & Ramsey Counties
Assertive Community Treatment (ACT) is an evidence-based, intensive “hospital without walls” model for those with severe, hard-to-manage symptoms. Our multidisciplinary team provides mobile services directly in Dakota and Ramsey Counties to decrease or prevent recurring acute episodes of illness and ensure community stability.
Youth ACT
Our specialized team provides Youth Assertive Community Treatment (ACT) and Individualized Rehabilitative Mental Health Services (IRMHS) for youth (ages 14-20) with serious mental illness. We focus on early intervention to prevent disabling conditions and promote successful transition to adulthood through community-based treatment.
Targeted Case Management
Targeted Case Management (TCM) provides focused assistance to individuals in gaining access to necessary medical, social, education, vocational, and financial services to support their mental and physical health. Our services are personalized to help clients achieve housing stability, manage finances, and successfully navigate complex benefit systems.
