Case Management & Community Treatment
Community-based, mobile support (Assertive Community Treatment/ACT and Targeted Case Management/TCM) helps individuals achieve long-term stability in the community they call home.
Crisis Support & Residential Treatment
Intensive Residential Treatment Services (IRTS) and short term crisis stabilization is a structured, comprehensive, 24-hour mental health residential program for people experiencing acute mental health challenges.
Supported Employment
We use the evidence-based Individual Placement and Support (IPS) model (intensive, customized, and time-unlimited support) to help clients find and keep competitive, meaningful work that aligns with their personal goals.
Homeless Outreach
Mobile teams compassionately meet unsheltered individuals wherever they are in the community to provide immediate essentials and connect them to benefits, safe housing, and mental health care.
Supportive Housing
Provides rent subsidies and case management services to people with disabilities and those experiencing homelessness, fostering long-term stability, independence, and full recovery.

- Who does Guild help?
Guild primarily serves adults (age 18+) and transition-age youth (ages 14–20) who are living with serious and persistent mental illness (SPMI) and/or experiencing chronic homelessness in the Twin Cities metro area (Dakota, Ramsey, Hennepin, Scott, and Anoka counties). Our integrated services—mental health, housing, and employment—are designed to serve those with complex needs who require a high level of coordinated support to achieve stability and recovery over their lifetime.

- What types of services are not provided by Guild?
Guild provides intensive community-based services, but we generally do not offer routine, long-term, weekly outpatient psychotherapy or traditional outpatient clinical appointments. We specialize in ACT (Assertive Community Treatment) which is a supportive mobile/community-based service and IRTS (Intensive Residential Treatment Services), which is a residential model of care. We focus on stabilizing crises, supporting housing, and securing and sustaining employment, often in partnership with other specialized outpatient providers who handle long-term individual therapy needs.
- Where is Guild located, and what is your service area?
Guild’s services are primarily mobile, meaning our teams meet clients where they are—in the community, in their homes, or in supportive housing—across the seven-county Twin Cities metro area (including Dakota, Ramsey, Scott, and Anoka counties). We have physical residential and crisis stabilization sites (IRTS) in Scott and Dakota counties. Our employment and case management teams travel extensively to ensure convenient, accessible support.
- How are Guild's services funded? Do clients have to pay?
Guild is a 501(c)(3) nonprofit. Our programs are funded through a blend of county contracts, state funding, and generous private donations. For clients, many of our services are covered by
Medical Assistance (Medicaid) or other public health insurance programs. Our intake team helps to confirm coverage and explain any potential out-of-pocket costs. We work hard to keep services accessible.
